She was made that way for the US original release because the US didn't want to show females getting beaten on, for the most part. So her being transgender sort of stuck as a cult following and got incorporated into it. Transgender though, not a man. There are boobs there.

If you think of it, there are some weird stories in videogames... ... Never thought the Poison gender one would ever come up here - and didn't mark the male checkbox -, but it was fun. xD
