That, my good Dude-Meistro, is a gender-bent rule34 version of Grim Matchstick from Cuphead, in the final phase of their bossfight, smothering the protagonist 'Cuphead' between the breasts that I definitely don't remember them having in game - while one of them drinks his moonshine through the straw poking out of his cup-head. So long as one of them doesn't turn their skull into a flamethrower, I think that about covers it by way of explanation.
